Free $500 Visa Gift Card

For chairs purchased from  May 1st – June 19th , we are offering a $500 Visa Gift Card with the consumer purchase of a Novo XT.

Please Note:

1. A serial number is required to validate your chair purchase, so your Novo XT must be delivered and received to fill out this form.
2. Only valid for consumer Novo XT purchases made between  May 1 and June 19, 2017  from a qualified Human Touch retailer.
3. You must provide the Serial Number found on the back of the chair backrest within 30 days after the delivery of your chair . Serial Number provided after 30 days of delivery will not be processed.
4. Not valid on the Novo massage chair.
5. Not valid with any other Novo XT offer.
6. Please allow 8-12 weeks for the delivery of your Visa Gift Card.

Buyer information will be used for mailing your card or contacting you if we have any questions. Also, the First and Last Name provided will be the name printed directly on the Visa Gift card.
